NIV Form DS-160

The U.S. State Department’s Bureau of Consular Affairs developed the DS-160 web-based nonimmigrant visa (NIV) application to incorporate all existing NIV applications into one interactive, online form.  It replaces the three forms that made up the Electronic Visa Application Form (EVAF) – DS-156, DS-157 and DS-158.

Applicants will continue to complete visa application forms on-line, as they did previously with the EVAF. We now provide “pop-up” translations of questions in many widely spoken languages, but the form must still be completed in English. With the new form, the applicant will have to upload a visa photograph, and submit the form via the Internet. The applicant will print just one confirmation sheet upon completion of the DS-160: https://ceac.state.gov/genniv

All applicants must fill out a new form DS-160 for each application, regardless of age. If you are planning to reapply or apply at a later date, you may not bring a DS-160 used for a previous application. However, you may reuse information from a previous DS-160 if that information is still current.  Applicants who appear at their interview with their old/ used DS-160 application forms will not be interviewed. When applying with a family or as a part of a group a separate DS-160 application should be completed for each family member (including infants) or for each individual within the group regardless of applicant’s age.
