Exception Request

The U.S. Centers for Disease Control and Prevention (CDC) will adjudicate and approve or deny all requests for exception to the vaccination requirement for humanitarian or emergency reasons.  To request an exception, please complete the below information for yourself and any relevant travelers.

Please email your information to ConsularTashkent@state.gov and include in the email subject line “Vaccination Exception Request.” U.S. Embassy Tashkent will send your request to the CDC for their review and decision.

1. Please select the type of humanitarian or emergency request:

  • Exemption from predeparture testing requirement; and/or
  • Exception for requirement for proof of being fully vaccinated

2. Complete the following information:

  • Last Name, First Name
  • Passport Number
  • Nationality
  • Cell phone number, including country code
  • Email address of passenger or head of household
  • U.S. address
  • Is the U.S. address a home address? (Yes or No)

3. Complete information about your flight:

  • Airline name:
  • Flight Number
  • Departure Airport:
  • Arrival Airport:
  • Departure Date:
  • Arrival Date:

4. Complete information about your vaccination status:

  • Fully Vaccinated
    • Name of vaccine product:
    • Date of first dose:
    • Date of second dose (if a two-dose series):
    • PDF or photograph of vaccination record
  • Not Fully Vaccinated

5. Provide brief explanation of why urgent travel is needed and how travel will contribute to health and safety of passenger(s)

6. As applicable:

  • Provide justification for humanitarian exemption for testing requirement
  • Provide justification for humanitarian or emergency exception to providing proof of being fully vaccinated

Provide documentation to support justification for exception (e.g. medical record or orders for medical evacuation).  Provide information regarding any other solutions that were sought prior to application (e.g. flight changes, testing en route, assistance in obtaining testing / vaccination, etc.)
